Name change applications

We provide notarization service for name change applications. For this purpose, the applicant (person completing and signing the name change application) has to visit our office and sign the document in front of a Notary Public. For identification purposes, the Notary will require you to present a Government issued photo identification document such as a driver licence, passport, PR card, citizenship certificate or other similar document. The document must bear your full name and photograph. By law, the person having their signature notarized must physically appear and sign in front of the Notary.

We also offer Mobile Notary Service, in case you are unable to visit our office. Presently this service is only available in the GTA area with prior appointment. For further details please go to Mobile Notary page of this website.
