Passport – In Lieu of Guarantor Form

The Canadian Passport application requires that applicant find a guarantor who has known the applicant for at least 2 years. If you can’t find such a guarantor, you have the option to complete and sign the “In Lieu of Guarantor form”. This is form is only available at passport office. A Notary Public is required to commission this form. If you are mailing in your application you will need certified copies of your supporting documents. We’d be happy to notarize this form in our office.

For this purpose, the applicant (person completing and signing the Passport application) has to visit our office and sign the statement in front of a Notary Public. For identification purposes, the Notary will require you to present a Government issued photo identification document such as a driver licence, passport, PR card, citizenship certificate or other similar document. The document must bear your full name and photograph. By law, the person having their signature notarized must physically appear and sign in front of the Notary.
